The Notice of Privacy Practices (NPP) explains how UCSF Health uses, discloses, and protects your health information, and describes your rights under HIPAA.
Under the UCSF Notice of Privacy Practices, Health Information Management Services (HIMS) is the primary contact to inspspect or obtain a copy of your medical record, as well as other HIPAA rights requests such as an Accounting of Disclosure.
